Healthworx

As a Grow Developmental employee, you have access to our HealthWorX Plan, a comprehensive healthcare plan with various levels of coverage.

Here's a quick breakdown:

Health Risk Assessment (HRA): We ask health-related questions and conduct tests to get a clear picture of your physical and mental health status.

Chronic Condition Management: We use your HRA data to create personalized plans for managing any chronic health conditions you might have.

Minimum Essential Care (MEC): You get unlimited preventative care, primary care, and specialist visits with minimal co-pays or deductibles.

Plus, there's a prescription discount program to save you money on medicines.

Additionally, your dependents can benefit from free homeschooling and a physical fitness program.

We also provide unlimited telemedicine consultations through TeledocX International at no additional cost.

*Please remember, this plan doesn't cover hospital visits. But, you can purchase separate indemnity programs if you need such coverage.

As long as you're working 20 hours or more per week and you're not on Medicare, you're eligible for this plan. And the best part? It's free for you.

If you wish to add dependents, it will cost $65 per month for each dependent.

You have the flexibility to opt out within 60 days of enrollment or in the case of a qualifying life event.

If you want to opt out or need more information, just call our bilingual call center.

If you already have a major medical plan, this plan can complement it and potentially reduce your costs.

Alternatively, you can choose HealthWorX Plan as your primary healthcare plan.

Our plan covers services from any doctor that accepts Medicare. You can enroll at any time, not just during major medical open enrollment.

Lastly, you should know that our partner, Xtension Health, provides some services for us, like Health Risk Assessment and Chronic Condition Management.

Dental

Principal Dental

Low Plan
The low plan has a $50 deductible, a $750 annual maximum benefit. It pays 100% of preventative services, 80% for basic but Major services are not covered.

High Plan

The High plan has a $50 deductible $2000 annual maximum benefit. It pays 100% of preventative services, 80% for basic and 50% for Major services. Child orthodontia is also included in this offering.

Willamette Dental

The Willamette Dental plan requires you to access care at a Willamette Dental Group facility.

This plan does not limit you to an annual benefit maximum like most traditional dental plans.

Services at a Willamette Dental Group facility are covered at a flat copay amount so you know exactly what your cost will be, for instance for all preventative services there is a $20 copay, a fill is $25 copay and a crown is a $350 copay.

There is Orthodontic Coverage available for adults and children too!

Accident Insurance, Critical Illness Insurance & Short-Term Disability

Accident Insurance

Even with a good health insurance plan, a trip to the doctor or hospital can be expensive. Many people find themselves paying more out of their own pocket each year. If you or someone in your family is hurt in an accident, the last thing you want to think about is how you are going to pay for medical care.

Accident expense insurance provides peace of mind and gives you additional cash to help pay your health insurance deductible and other expenses.

Key Features:

Unlimited lifetime maximum benefit with no age-related benefit reductions.

Benefits are paid based on the schedule of benefits provided for each covered accident.

Waiver of Premium: Premium is waived following a 60-day period of disability due to a covered accident for as long as the covered person remains disabled.

Portability is included.

Critical Illness

More people are surviving life-threatening illnesses than ever before. Unfortunately, the cost of critical illness care is high and medical bills can follow survivors long after they’ve proven victorious in their fight.

Critical illness insurance provides peace of mind and gives you additional cash to help pay your health insurance deductible and other out-of-pocket expenses.

Critical Illness insurance pays a lump-sum benefit directly to you if you are diagnosed with a stroke, heart attack, or a number of other covered conditions.

Key Features

Unlimited lifetime maximum benefit.

No age-related benefit reductions.

Additional Occurrence: Additional occurrences of other specified diseases are paid at 100% of the payable lump-sum benefit with no waiting period between diagnosis.

Reoccurrence: A new diagnosis of the same specified disease for which a benefit has already been paid is paid at 100% of the payable lump-sum benefit.

There is a 6 month waiting period between diagnoses to be eligible for a reoccurrence payment.

Waiver of Premium: Premium is waived after 60 days of disability due to a critical illness and up to 24 months.

Portability is included.

12/12 pre-existing condition limitation.

Term Life Insurance

Term life insurance is an important tool for financial security. Choose between Term 10 and Term 20 policies.

If you were to pass away during the covered term, we would provide a lump-sum payment—known as a death benefit—to your chosen beneficiaries.

Term 10 Life Insurance: This policy provides coverage for ten years. If anything should happen to you within this decade, your beneficiaries will receive the policy's death benefit.

Term 20 Life Insurance: This policy extends your coverage to 20 years.

If you were to pass away at any point during this period, your beneficiaries will be granted the death benefit.

Our term life insurance policies are uniquely designed for affordability and convenience.

While they do not accumulate cash value, they do provide key benefits:

Portable Coverage: If you leave or change jobs, you can take your policy with you.

Payroll Deduction: Premiums are automatically deducted from your paycheck for hassle-free payment.

Family Coverage: Depending on the plan, you may be able to secure coverage for your spouse and/or dependent children.
